Clay County is a commuter county, and that shapes the insurance question more than people expect. More than half of the people who live here work somewhere else, the highest rate of any county in Florida, and the average commute runs close to 33 minutes. Plenty of those people work for themselves, and when you work for yourself nobody hands you a benefits packet in October.

Check your hospitals before you check the price
This is the mistake I see most often, and it is the expensive one. People sort plans by premium, enroll, and find out in March that the hospital ten minutes from the house is out of network.
Three hospitals serve Clay County directly:
- HCA Florida Orange Park Hospital, on Kingsley Avenue in Orange Park
- Baptist Medical Center Clay, in Fleming Island
- Ascension St. Vincent's Clay County, in Middleburg
If you commute into Duval, you may also want Jacksonville hospitals in network, because that is where you are if something happens on a Tuesday afternoon. Two plans that look the same on a comparison screen can treat those systems completely differently. Checking your actual doctors and hospitals against the network is part of every quote I run, and it takes me minutes.
Where you live is what prices your plan
Plans and prices are set by the county on your application, not by where you work. A neighbor in Fleming Island and a coworker in Riverside can be looking at different plans at different prices for the same family situation. The same split runs through St. Johns County, where the hospital people actually use is often up in Duval.
It comes up constantly with commuters, usually as "my buddy at work pays less than that." He might. He also might live in a different county.
The Medicaid gap, said plainly
Florida never expanded Medicaid. That leaves a band of income where you earn too much to qualify for Medicaid and too little to qualify for a premium tax credit on the marketplace. Nothing catches you there.
For a salaried household this rarely matters. For someone self-employed whose income swings by season, it matters a great deal, and the fix is usually in how you estimate income at application rather than in which plan you pick. That is a conversation worth having before you enroll, not in April when the return is being filed.
If your family is connected to the military
A lot of Clay County households have a service connection, between Camp Blanding and the bases just up the road in Duval. If you are on TRICARE, you generally do not need me, and I would rather say that than sell around it.
Where I am useful is the edges. A spouse with self-employment income of their own. Someone recently separated from service who is not on an employer plan yet. A young adult aging off a parent's coverage, which has its own set of rules and deadlines.
The dates
Florida uses HealthCare.gov, so open enrollment for 2027 coverage opens November 1, 2026. It closes January 15, 2027, which CMS confirmed in August, though the safe move is still to be enrolled by December 15 rather than waiting. My open enrollment guide has the current state of it.
Outside that window you need a qualifying life event: losing job coverage, moving, marrying, or having a child. Moving into Clay County counts, which is worth knowing if you are one of the many people who just did.
